METRO ADULT SOCCER TERMS OF REGISTRATION AND RELEASE OF LIABILITY

1. Registration. I am the above named player and I am hereby registering to try out for and/or play soccer with Metro Adult Soccer ("MASL"). I understand that this registration is not effective until accepted by MASL and that MASL may decline to accept this registration for any reason not in violation of the law. Upon acceptance of this registration by MASL, this registration becomes a contract under which MASL agrees to permit me to participate in a Metro Adult Soccer program, in accordance with MASL's rules, and I agree to be bound by these Terms of Registration and the MASL's rules and policies as set forth in MASL's FIFA Laws of the Game and Policy Guide. I agree that this registration form is binding upon me throughout the duration of my MASL membership and can apply to one or more of the following MASL programs: MASL sponsored Winter Camps, Spring League, Beginner League, Summer League, Fall League, Soccer School (winter or summer), MASL sponsored vacation tournament, MASL sponsored exhibition game, MASL sponsored playoff tournament or any other MASL sponsored program or event. Unless MASL notifies me to the contrary, I understand that this registration is deemed accepted by MASL upon MASL's receipt of same together with the required payment in full. MASL reserves the right to terminate any player's participation MASL activities at any time during the course of that activity, session or season, for any reason which is not in violation of the law. A pro rata refund will be made, based upon remaining games or sessions, to any player whose participation is so terminated without cause (i.e. not on the basis of a violation of these Terms of Registration of MASL's rules, policies, directives, or decisions).

2. Payment. I understand and agree that the full cost (including any past due or balance forward fees/charges) of the season/activity for which I am registering is required to be paid by me to MASL. I further understand and agree: a. that I will not be permitted to play or otherwise participate in any MASL activities unless the full season/activity fee is received in the MASL Office before the season/activity begins; b. that any payment not made on-line to MASL may delay my payment being credited against my account and may cause late fees or fines to be assessed; c. that any payment not made in full before the season/activity begins will be subject to a twenty-five ($25) dollar late payment processing fee for each month or portion thereof that any payment or portion of payment remains due and unpaid. The twenty-five ($25) dollar fee shall be charged on the first day of each month payment is not received in full, beginning with the first full month of the season/activity; d. that full payment due remains due regardless of my participation or continued participation, whether I choose not to participate, cannot participate for personal or business reasons, or whether I am suspended (e.g. for nonpayment of fees or for breach of MASL's, policies or directives) unless a proper and timely request for refund has been submitted in accordance with the section on "Refunds" below; e. that the cost of the season/activity is solely for field time, game officials, and scorekeeper charges, and for organization and administration expenses of MASL and it does not include the cost of any other related expenses such as required uniforms, supplies, equipment, or independent practice time; f. that first time player's cost of registration shall include a nonrefundable registration processing fee of twenty five ($25) dollars which will be charged (and not refunded) regardless of whether I participate in the season/activity. g. that each check submitted for any MASL season/activity returned due to insufficient or unavailable funds shall be subject to a fifty ($50) dollar charge each time the check is so returned; h. that I will pay all reasonable costs incurred by MASL in collecting any amounts due thereunder, including but not limited to collection costs, court costs, and attorney's fees; and i. that I will reimburse MASL in full, within ten (10) days of notice, for the cost of any property damage for which I am deemed responsible by MASL or for which MASL is billed by any soccer field/park facility.

3. Refunds. I understand that MASL will not provide refunds for any reason. I may transfer my money to another player with a fifty ($50) dollar administration fee. Any request for a transfer must be made in writing, sent by certified mail, to the MASL office and be received by MASL prior to the deadline date relating to the season/activity. The uniform fee may not be transferred after the orders have been placed. 

4. Prorating. I understand that League fees shall not be prorated unless: a. I am injured during the season; or b. I am a returning player and my team already has more fully paid players than are required by MASL. In either circumstance, the prorate shall not be for more than five (5) games/sessions.

5. League Rules. I have received a copy of, and have read and understand all the provisions of, these Terms of Registration. I further understand that a copy of the MASL FIFA Laws of the Game and Policy Guide is available to me, completely free of charge, at any time by contacting my captain or by contacting MASL at the telephone number listed above. I agree to abide by all rules, policies, directives, or decisions of MASL, whether presently in force or adopted by MASL in the future, including MASL's rules that all players: a. must wear MASL approved protective shin guards and other protective equipment at all times while on the playing field, sidelines or player's bench, whether or not play is under way; b. shall conduct themselves in a mature and responsible manner which will reflect favorably upon MASL;
c. shall never participate in any game or MASL activity while under the influence of alcohol or any drug or intoxicating substance, and never bring any alcohol, drug, or intoxicating substance on the premises (including the parking lot) of any Soccer field/park facility for any reason; d. shall comply with MASL's color designations relating to his or her team; e. may be expelled without refund from MASL for failure to comply with the Terms of Registration, any MASL rule, directive, or decision, or for any reason of the following reasons; 1. failure to make full payment when due thereunder; 2. falsification of registration information; 3. receiving three (3) red cards within a period of twelve (12) calendar months; 4. other misbehavior as defined in these Terms of Registration and/or the MASL's FIFA Laws of the Game and Policy Guide, directives, or decisions. 5. Failure to remit full payment as part of the team's payment obligation for open roster spots and / or team member playing fees (as part of meeting the minimum revenue requirement as outlined to all captains prior to each season's start).

6. Assumption of Risk. I UNDERSTAND AND AGREE THAT THE SPORT OF SOCCER HAS INHERENT RISKS OF PHYSICAL INJURY, INCLUDING SERIOUS BODILY INJURY, PERMANENT DISABILITY, PARALYSIS AND DEATH, AND I ACCEPT AND ASSUME ALL SUCH RISKS. These risks and dangers may be caused by my own actions or inaction's, the actions or inaction's of others participating in the event, the conditions or layout of the event, or my or other's equipment. There may be other risks not known to me or that are not readily foreseeable at this time. The social and economic losses and/or damages that could result from those risks could be severe and could permanently change my future. I HEREBY RELEASE, DISCHARGE AND COVENANT NOT TO SUE MASL, its affiliated clubs, their respective administrators, directors, agents, coaches, and other employees of MASL, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners/proprietor and/or leasers or their employees of any premises used to conduct MASL games and/or events FROM ALL LIABILITY TO ME, my personal representative, assigns, heirs, next of kin, FOR ANY CLAIMS, DEMANDS LOSSES OR DAMAGE ON ACCOUNT OF ANY INJURY sustained by me while participating in any MASL activity, including, but not limited to, death or damage to property, CAUSED OR ALLEGED TO BE CAUSED IN WHOLE OR IN PART BY THE NEGLIGENCE OF ME OR OTHERWISE. I hereby certify that I have adequate medical insurance to pay for the treatment and cure of any such injury which may result from my participation in MASL activities, and I agree to be solely responsible for all medical, and other, costs arising from any injuries I may receive while participating in MASL activities. I further certify that I have no known medical condition which would prohibit me from participating in the sport of soccer. I acknowledge that MASL strongly recommends each player to receive a physical examination by a competent physician prior to participating in any MASL activity. I further agree that prior to participating in any MASL sanctioned event, I will inspect the playing field, facilities and equipment to be used, and if I believe that anything is unsafe I will immediately advise my coach, team captain, and the game supervisor of such conditions, and I will refuse to participate until the unsafe condition(s) has been removed. I further understand that MASL does not warrant any equipment used in any MASL activity or any facility at which MASL activities are held.

7. Additional Terms. I further understand and agree to the following: a. MASL's decision shall be final on the adoption, enforcement, and interpretation of all FIFA Laws of the Game and policies, and all other matters relating to MASL activities. I agree to abide by all such decisions not withstanding my personal preferences to the contrary.
b. MASL will be the sole authority as to what level and what team I will be permitted to participate. c. Each team will have a team captain who will be elected from among the team's players at the beginning of the season. The team captain will be authorized to act as the sole agent of the team for all purposes relating to MASL activities and events. If I am elected team captain, I will act to ensure that the team strictly complies with all MASL rules and policies. d. MASL may use for promotional, advertising, or educational purposes, without notice or compensation to me, any audio and/or visual recordings of any MASL activity in which I may appear. e. "MASL," "MAS," "Metro Adult Soccer," "Metro Adult Soccer League," "Continental Cup," and "Get in the Game," and the names of and rights to all MASL teams, are the exclusive property of MASL and shall not be used in any manner in connection with any non-MASL play or any tournaments or multiple games that have not been expressly approved or endorsed by MASL. f. In conjunction with the MASL FIFA Laws of the Game Policy Guide, these Terms of Registration, and any written notification of playing rules, policies, or conditions, represent the entire agreement between MASL and me, and there are no valid or binding representations, inducements, promises, or agreements, oral or otherwise, between MASL and me that are not set forth above. This agreement may be amended or modified only by writing signed by me and an authorized corporate officer of MASL. To the extent there exists any inconsistency or conflict between these Terms of Registration and any other statement of MASL policy or rules, these Terms of Registration shall prevail and shall remain fully binding upon me.
